Customizable Onboarding Fields - Questionnaire/Form Potential

Some clubs require some type of simple form for users to complete on initial signup. This would be separate from the club wavier, and more for simple questions. Question types could potentially include:

- Yes / No
- Checkbox
- Short answer (Text/Number)
- Dropdown

Two more use cases would include:
- A field for another users name for referral purposes
- A “how did you hear about us” field or drop down

Consider the ability to rename the existing fields.

    would be VERY helpful. We keep track of things like “Member-Guest” visits. Or “Out of Town Visitor”, etc. If you wanted to get fancy with this, make these selections exportable through your API to be used as a trigger for a specific follow-up automation based on the selection

    I’d like to ask where users found out about us. That isn’t currently a possible onboarding question, right?
