Allow both appointment resources to be selected by users

Currently, when an appointment type requires multiple resources, members can only choose the primary Host. Everything under "Other Resources Required" is auto‑assigned from whatever's free.

Some clubs want members to be able to choose two resources at booking time, not just one.

Example use case: a member has both a preferred instructor and a preferred facility, and wants to select both when they book.
